Obtaining Medical Records
The Medical Records Department can provide information on children’s immunization status, verify work excuses for employers, prepare insurance and disability claim forms, and handle requests to transfer health records to or from the clinic. To make use of these services, stop by the medical appointment desk and ask to speak to someone from Medical Records or call 799-5768 during regular clinic hours.
You can obtain medical records by printing and completing the authorization forms and mailing or faxing it to the Menominee Clinic. You may also contact us if you have questions about obtaining medical records.
