FAQ's

You Have the Right too....


               

1.     to considerate and respectful care. 

2.      to obtain from your physician or health care provider complete and current information concerning your diagnosis, treatment, and prognosis in terms you can understand.  

3.      to receive from your physician or dentist information necessary to give informed consent prior to the start of any procedure and/or treatment.  

4.      to refuse treatment to the extent permitted by law, and be informed of the medical consequences of your actions. 

5.      to every consideration of your privacy concerning your personal health care program. 

6.      to expect that all communication and records pertaining to your care should be treated as confidential. 

7.      to have the tribal clinic, within its capacity, make reasonable response to your requests.

8.      to be advised if the tribal clinic proposes to engage in or perform human research affecting your care or treatment. 

9.      to be referred to specialized care if needed.  

10.   to expect reasonable continuity of care and education. 

11.  to know what clinic rules and regulations apply to your conduct as  a patient

 You are Responsible......


  1. for informing your health care provider if you do not understand instructions.
  1. for informing your health care provider if instructions cannot be followed and to assist with a more suitable plan.
  1. for telling your health care provider about past illnesses, hospitalizations, medications and other matters affecting your health.
  1. for notifying your health care provider about any changes in your condition.
  1. for notifying your health care provider if treatment of medications or other modalities do not help with your condition.
  1. for cooperating in observing safety regulations and policies (smoking in areas and children staying with parents in designated areas.)
  1. for being considerate of clinic personnel who are involved in providing your health care.
  1. for appropriate and respectful use of clinic facilities and equipment, to assure its availability to future patients.
  1. for personal valuables and belongings.
  1. for making appointments for health care whenever possible.
  1. for canceling appointments, if necessary, early enough to allow time for others to schedule appointments.
  1. for bringing in your insurance cards and keeping us informed of any medical assistance you may have or may be getting.
  1. Parents or guardian must accompany minor children to the clinic or hospital.
