What services does Community Health Nursing Offer?

               

Home visits.  Skilled home nursing visits are available seven days a week.  Referrals are accepted from physicians, hospitals, family members or patients.  Call 799-3871.  

Well child screenings/healthchecks.  Patients from birth to age 20 are eligible for comprehensive physical exams including health and vision screening.  These physicals are accepted for WIAA sports, Head Start, day care, boarding school and camp physicals.  To schedule your child, call 799-3871. 

Immunizations.  Immunizations for children and adults are available on a walk-in basis.  If you have questions about your children’s status, we can check the record and make copies for you if necessary. 

Loan closet.  Wheelchairs, walkers, and commodes are available on a first-come, first-served basis for short-term loan.  For more information, call 799-3871. 

Blood pressure screenings.  Blood pressure checks are available on a walk-in basis.  See the CHNS receptionist.  Blood pressure screenings are also done at senior centers on a regularly scheduled basis. 

Flu shots.  Flu shots are available during the regular flu season.  Register first at patient registration. 

Rural infant health program.  The Rural Infant Health program reaches out to identify and serve “at risk” or “high risk” infants from birth to age 3.  The program provides transportation to medical appointments if no other transportation is available, and assists with childhood immunizations.  For information, call 799-5438. 

Transportation.  The Community Health Department has two transporters available.  Transportation is provided to patients who are referred to outside specialists by the Menominee Tribal Clinic providers.  They also provide transportation to the Clinic for residents who have no other means of transport.  Rides are available on a first-come, first-served basis and it is best to book rides ahead of time.  Call 799-3871 or 799-5430.

Adolescent Health.  Adolescent Parent Self-Sufficiency provides parenting classes, transportation, job shadowing and a Prevent Another Pregnancy Program to teen parents 19 and under.  An Adolescent Pregnancy Prevention Program offers a peer monitoring program, case management services and education with a goal of lowering the teen pregnancy rate on the reservation.  For information, call 799-5754.
