Contract Health Services covers health care services provided by doctors, hospitals, dentists, optometrists or pharmacies outside of the Tribal Clinic. 

Contract Health Services will pay those costs only if no other funding source is available and a patient has met all of the other requirements. 

To be eligible, your primary health care provider must be the Menominee Tribal Clinic, and a Menominee Tribal Clinic provider must refer you for treatment.  You also must also fill out an application to Contract Health Services to determine your eligibility. 

Notify Contract Health Services at least three days before your appointment.  Without prior approval, requests for payment will be denied.  In cases of emergency treatment, notify Contract Health Services with 72 hours afterward. 

Appeal procedure.  Patients who are denied Contract Health Services will be notified in writing with the reasons for denial, and will have the opportunity to appeal the decision to the Health Committee.

Benefit Specialist Program.  Benefit Specialists are available to assist persons in applying for healthcare benefits such as Medicaid, BadgerCare, Wisconcare, etc. 

In many instances, applications can be started in the Contract Health office.  Persons having questions or problems with the application process, or are in need of transportation to apply for the benefits, call Benefit Specialists at 799-5766 or 799-5857.
